Digital reporting changes the economics: when reporting takes seconds from a phone, people report more. Rising near-miss numbers in the first months are usually a sign of a healthier culture, not a worse site.

The key is to close the loop. Every report needs an owner, an action and a visible result, or people stop reporting.
